REQUIREMENTS (HOSPITALS, CLINICS, MATERNITY HOMES AND NURSING HOMES)

REQUIREMENTS FOR REGISTRATION (FOR HOSPITALS, CLINICS, MATERNITY HOMES AND NURSING HOMES)

  1. Building Plan showing dimensions, etc. and uses of room.
  2. Three (3) years tax clearance certificate.
  3. Letter of application for registration.
  4. Letter of Acceptance from three (3) qualified Nurses/Midwives who have agreed to work with you. They should also provide their certificates and current practicing license
  5. Your Professional certificates. For Specialist hospitals to include a specialist certificate(fellows).
  6. NYSC Certificate or its equivalent.
  7. Current practicing License.
  8. Supervising Doctor’s certificates (for Maternity and Nursing Home only).
  9. Certificate of full registration.
  10. Evidence of membership with a professional Association, for Doctors NMA, AGPMPN.
  11. Central clearance letter from the state NMA indicating clearance from zonal level where the Hospital is located (for Hospitals only).
  12. Certificate of registration with CAC.
  13. Two (2) passport photographs.
  14. Number of staff employed.
  15. One office flat file.
  16. Health Service Data (NHMIS) for Registration/Renewal (for Hospitals only).

NB: For Hospitals that are not owned by Medical Doctors, a qualified Medical Doctor with 5 years and above experience can only use his/her certificate once for a period of one year which is subject to renewal and once there is any change, please notify the Department of Medical Services
