REQUIREMENTS

MINIMUM REQUIREMENTS FOR LISTING OF COMPLIMENTARY/ ALTERNATIVE MEDICINE PRACTIONERS

  1. Provide an Office space.
  2. Tax Clearance Certificate.
  3. Letter of Application for Registration.
  4. Letter of Acceptance from one qualified nurse who has background experience in alternative medicine and has agreed to work with you, the certificates and current practicing license.
  5. Provide evidence of co-operate affairs commission (C.A.C) registration documents.
  6. Provide evidence of registration with your association and clearance from your association.
  7. WAEC Certificate/Equivalent.
  8. Provide evidence of certificate of formal training in complementary medicine/alternative medicine.
  9. Show evidence of workshop or other relevant trainings attended.
  10. Two passport photograph.
  11. One office flat file.

NOTE: You are allowed to market or dispense only NAFDAC approved herbal/alternative medicines.

Show evidence of commitment to avoid, prevent and stop propagation of health/herbal information/benefits through public address system and healthcare facilities.
