REQUIREMENTS FOR REGISTRATION OF MEDICAL LABORATORIES IN ANAMBRA STATE

  1. Building plans showing dimensions and uses of room (3 Copies).
  2. Three years’ tax clearance.
  3. Application.
  4. Your professional certificates.
  5. NYSC certificates or its equivalents.
  6. Current practicing license.
  7. One file jacket with tag.
  8. Certificate of Registration with CAC.
  9. Number of staff employed.
  10. Two passport photograph.
